For a correction

Include the card name, a link to the official issuer terms and a short explanation of the discrepancy. Offer details can vary by application channel, so the exact source helps.

For a privacy request

Choose Other and state whether you are asking about access, correction or deletion. Include enough information to identify your prior feedback and an optional reply email. Do not send identification documents, account numbers or bank login details.
